We know it’s important for your residents to be treated close to home. That’s why we want to be your in-house rehab provider. This way, your residents can return home after a surgery, accident or illness and get help from the right therapists with the right equipment right where they are, seven days a week.

Team Care Rehab therapists aren’t just visiting to provide therapy, we’re members of your community.  Being an in-house therapy provider allows our same therapists to treat the same residents from start of care to discharge, so it’s always the same smiling therapists that your residents interact with. We also lead exercise classes to engage the resident’s physical health while also identifying any problems that may arise.  We can provide gait-training, fall prevention, and other training to your staff to ensure your residents receive the highest quality of care. If the facility is Type-A, we can work with the nursing staff to ensure every resident is able to meet the requirements of your facility. We provide all this at no cost because our goal is to have your resident live life comfortably in your facility for as long as possible.

We supply all the equipment needed, including mat tables, weight machines and bikes. We’ll move into any available space and help your residents continue living their best life – without having to drive anywhere.
